Staff Compliance Checklist

Agency staffing coordinators: Please use this checklist to confirm all documents required in the compliance package have been submitted.

Please direct your Agency staff to the webpage below to complete their pre-onboarding tasks and electronically sign the acknowledgment forms. When the staff returns these documents to you, please forward to the hospital location’s HR team, along with the COA (Certificate of Agreement)
