Agency & Contracted Services staff are a vital part of the care provided at Landmark Hospitals, so it is critical to have staff members well-oriented to our hospital processes.

Please review the materials below and complete the acknowledgement forms. You may either sign electronically (use Adobe to electronically sign the pages in the downloaded PDF), or print the PDF file and manually sign. In either option, please return to your contracting agency with your signatures as soon as possible to avoid delays in onboarding.

Your signature on the onboarding and acknowledgement forms attests that you have reviewed and understand these policies and processes. Please raise any questions you have with your on-site supervisor.

To expedite onboarding, please review the material and sign the acknowledgement forms as soon as possible.

REVIEW and SIGN the Employee Health forms (and optional declination forms for medical or religious reasons; to decline the annual flu vaccine, you may use either form. Declining the flu shot does not require additional medical verification.) - (Updated 9/2025)

FORWARD the signed acknowledgement bundle to your staffing agency contact, who will forward to the Landmark Hospitals HR team

PROVIDE the additional documentation required in the compliance package (the agency has a checklist of documents needed, such as BLS, ACLS, Immunization records, transcript or diploma for RT, etc.).

If you have a preferred photo to use on your badge, please forward through your agency so we can have your badge ready when you arrive, or we can take a photo for your badge on your first day.
