Clinical Students at Landmark Hospitals
Welcome Students! Landmark is honored to play a role in your clinical education. While you are with us, you will get to experience care provided to medically complex patients and observe how an LTACH (Long Term Acute Care Hospital) functions. LTACH are Critical Care hospitals providing speciality care to patients for an average of 25 - 30 days. LTACHs play a critical role in the continum of healthcare in the community. We do not have an emergency room or out-patient services; 100% of our patients are referred to us from other Acute care hospitals (typically ICU or CCU) or other healthcare facilities that have identified a patient needing a higher degree of care. Our patients can be discharged home or to their next level of care (Rehab, SNF, long term care facility).
As a student, there are certain expectations we have when you are in our facility, all of which begin with prioritizing the patient’s privacy, safety, and dignity. As a reminder, students may not take any paper or patient specific information outside of the hospital.
We have outlined key policies and procedures that will support success during your rotation @ Landmark Hospitals. Please review the materials below and complete the acknowledgement forms. You may either sign electronically (use Adobe to update the downloaded the PDF files) or print and manually sign them. In either case, please return the signed forms to your school contact who will in turn provide these documents, along with other credentials to the HR contact at Landmark.
Your signature on the onboarding and acknowledgement forms attests that you have reviewed and understand these policies and processes. Please raise any questions you have with your instructor.

REVIEW and SIGN the Acknowledgement Forms (updated 10/2025)
REVIEW and SIGN the Employee Health forms (and optional declination forms for Vaccinations & Immunizations) - (Updated 10/2025)
RETURN the signed acknowledgement bundle (and any other pre-requisite documents for the rotation (such as BLS, immunization records, etc.) to your school contact, who will forward all the student packages to the Landmark Hospitals HR team before the rotation start date.
We hope you will enjoy your time at Landmark and encourage you to CONSIDER A PART TIME JOB with us during school or after you graduate! We often have students work a PRN schedule (1 shift a pay period) to both earn extra $, but also gain skills while they are in school. If this is an interest to you, please stop by the HR office for more information.
